| Additional Requirements | • Complete a Volunteer Profile (attached) or follow this link https://form.jotform.com/200965879229268 • Review a Volunteer Orientation presentation (sent once I receive the completed Volunteer Profile) • Attend an interview (virtually) • Provide a Vulnerable Sector Check (I will provide the volunteer with a letter to obtain a record check) • Provide three references (non-family members) • Sign an Oath of Confidentiality • Sign a Volunteer Acknowledgement • All volunteers must be at least 18 years of age • Agree to a CPIN search (information attached) • Provide Covid-19 vaccination certificate (2 doses) |
