Description (Service) COVID-19 During the COVID 19 situation, check the website for change, cancellations and services available 
A non-profit, community governed, charitable organization that focuses on the health and well being of individuals in the community * Health professionals include: a physician, nurse practitioners, registered nurses, a registered dietitian, diabetes educators, a mental health and addiction counsellor, a physiotherapist, a health promoter and social workers 
Provides access to health care services, free community programs and community development * Also offers referrals to other local health services and personal development groups 
Priority will be given to individuals and families that are currently without a doctor, and who fall into one of the following priority populations: 
* Individuals or families experiencing poverty, housing and/or food security challenges 
* Youth, between the ages of 12 and 24, with an emphasis on at-risk youth 
* Isolated seniors 
* Individuals with disabilities that present barriers to health care service access 
* Individuals or families experiencing mental health and/or addiction issues.  
Note: This is NOT a walk-in clinic 
Services with a Nurse Practitioner are also offered to registered clients in Elmvale on Wednesdays by appointment at the Springwater Health Services Building (Kenzie Knox Building - 35 Queen Street West, Elmvale) from 9 am-3 pm, see separate entry for further details.

Public Comments From Jan 14, 2020 to Mar 31, 2020, a Nurse Practitioner will be available for people who do not have a primary health provider * Appointments are required and will be available Mon-Fri during normal business hours * To book an appointment, call 705-352-3471 * No walk-ins will be accepted

Hours SGBCHC: Mon, Wed, Thu 9 am-12 noon, 1 pm-4:30 pm * Tue 9 am-12 noon, 1 pm-7:30 pm * Fri 9 am-1 pm

Application For new patients in need of primary health care, call reception to get on the waitlist. Partner organizations can refer clients with more urgent needs by calling the main line.
