Description (Service)

Supports seniors and older adults to remain independent in their own home by providing three levels of assisted support:

Assisted Living Services provide support with personal care and hygiene, medication support, meal preparation and light housekeeping
Low Acuity Services provide personal care and hygiene support for one to two hours per per week
Homemaking Service also available - See separate record

Areas Served	Clearview (Simcoe County) ; Collingwood ; Wasaga Beach
Eligibility	Older adults with age related conditions, 55 year(s) and up.
Application	Call for more information. All referrals welcome for Assisted Living Services and Low Acuity Services. Call Paula Gueran, Community Support Supervisor for more program information.
