Patient-centered education and on-going management for individuals with Type 1 and Type 2 diabetes

patients receive initial assessment and are provided with follow-up appointments as indicated by their diabetes status
education focuses on supporting healthy lifestyle changes for improved diabetes self-management, understanding principles of healthy eating and physical activity, importance of blood glucose monitoring, understanding targets for control, role of oral medications and insulin
registered ADP insulin pump program site for Barrie and surrounding communities
