Adult Outpatient Assessment and Rehabilitation- Physiotherapy, Hand Therapy, and Rehabilitation Day Programs, and Swallowing Clinic

Rehabilitation Day Program is for patients with stroke within the last 3 months, or amputation requiring new prosthetic training
Rehab programs staffed by interprofessional teams of nurses, physiotherapists, occupational therapists, speech-language pathologists, and social workers
Swallowing Clinic staffed by Speech-language pathologists and radiologists

Public Comments	The entrance used will depend on which clinic you are attending- please follow the booking clerk's instructions Simcoe Entrance: Outpatient Rehabilitation Clinic (Physiotherapy, Hand Therapy, Rehabilitation Day Program) is located on Level 1, enter at Simcoe Entrance Main Entrance: Swallowing Clinic is located on Level 2 - Imaging Department, enter at Main Entrance faces Georgian Dr, on the north side of the health centre, open daily from 6 am-9 pm

Eligibility	Age Requirements: 18 years and up For information about specific program/clinic eligibility criteria, call department phone number
Application	Referral from a specialist physician (surgeon or physiatrist) is required for Physiotherapy & Hand Therapy Referral from a physician or nurse practioner is required for Rehabilitation Day Program and Swallowing Clinic Appointment necessary for all clinics/programs
