      • paper application forms can also be picked up at Service Canada Centres or Funeral Homes
      • don't delay in sending completed application forms, delay may cause a loss of benefits
      • proof of common-law relationship or marriage are required, see website for details
      • completed application forms and correspondence must be mailed or can be dropped off at local Service Canada Centre
