Description (Service)

Service that refers Ontarians who do not have a primary care provider (family physician or nurse practitioner) to one locally who is accepting new patients

priority is given to individuals who have greater health needs
waiting period depends on availability of family physician or nurse practitioner
Care Connectors are nurses who:
- review health information provided upon registration with the program
- follow-up with the individual with additional questions, if needed
- work with local family health care providers to determine who may be accepting new patients in the community
- once registered with the Health Care Connect Program an individual receives contact information for his/her assigned Care Connector

Public Comments	This agency is formerly known as North Simcoe Muskoka Community Care Access Centre (NSM CCAC)
Hours	Mon-Fri 8:30 am-4:30 pm
Areas Served	Grey County ; Muskoka District ; Simcoe County ; District of Muskoka - all areas Simcoe County - all areas north of Hwy 89 Grey County - small portion of eastern Grey County, including Maxwell and Feversham
Boundaries	District of Muskoka - all areas Simcoe County - all areas north of Hwy 89 Grey County - small portion of eastern Grey County, including Maxwell and Feversham
Eligibility	Must have valid Ontario Health Card
Application	To register call 1-800-445-1822 (have Health Card available) can also register online at www.ontario.ca/healthcareconnect by completing a health questionnaire to determine need for family health care services
Languages	English; French - Reception, Interpretation, services can be arranged in French
Fees	None - covered by OHIP
