Suggesting Updates to Database Listings

When viewing the full details of a community program, you will see the option to 'Suggest Update' just below the name of the organization and program. Clicking this option will allow a user to submit feedback to Community Connection's team of data curators. Users can also select 'Suggest Record' on top menu bar to suggest a new listing.

All patients attending the community nursing clinic will be assessed and provided with individualized instruction on self care and self-management supports

Application	Referral by Ontario Health atHome only. Patient will be contacted by Bayshore Home Health with appointment details. Patients can schedule an appointment at times that is convenient for them.
