Suggesting Updates to Database Listings

When viewing the full details of a community program, you will see the option to 'Suggest Update' just below the name of the organization and program. Clicking this option will allow a user to submit feedback to Community Connection's team of data curators. Users can also select 'Suggest Record' on top menu bar to suggest a new listing.

Description (Service)

A regional, primary health care team set up to provide collaborative health care to the Georgian Bay population. The team consists of Doctors; Nurse Practitioners; Registered Nurses; Occupational Therapists; a Health Promoter; Registered Dietitians; Mental Health Counsellors.

Area residents have access to GBFHT's services by getting a referral from their GBFHT Family Physician (only patients of affiliated physicians may be referred). Family physician referral also required for most clinics. GBFHT is the regional Diabetes Education Centre (DEC). All patients with diabetes regardless of who their family doctor is may be seen at the DEC.

Services include:

Mental Health Counselling and Support:

  • Just-in-Time Counselling (1-3 sessions) to address a single concern.
  • Brief Counselling/group (up to 8 sessions) to focus on a specific issue.
  • Complex Care where needs are complicated and not easily met by existing services:
    • Adult - counselling for stabilization, navigation to community, specialist or social services.
    • Children and youth - navigation and connection to community mental health and child development services.

  • Diabetes Care and Education
  • Nutritional Counselling
  • Chronic Disease Self Management Program
  • Smoking Cessation Program
  • Hypertension Clinic
  • Cardiac Care Clinic
  • Ontario Telemedicine Network participant

Falls Prevention: The family health team assists seniors with preventing falls or recovering from falls by providing:

  • fall risk assessment/screening at annual physicals including basic falls prevention advice
  • after a fall, identifying the cause of the fall and helping patients develop strategies to prevent future falls
  • referral to other falls prevention programs or services
  • regular medication reviews
  • basic vision assessments and reminders at annual physicals to have regular biannual eye exams

The Georgian Bay Family Health Team is a member of the Staying Independent Falls Prevention Coalition (SIFPC)

Eligibility Some programs or services require referral by GBFHT family doctors, call for more details.

Residency Requirements: The Georgian Bay Family Health Team (GBFHT) covers a 35 Km area around the Collingwood General and Marine Hospital (CGMH)
Application Appointment required. Physician referral is required for some programs or clinics.
