Description (Service) Medical care team focused on family and community health * coordinates patient care in order to assess and treat illness * team includes physicians, nurses, nurse practitioners, registered dietitians, pharmacists, respiratory educators, diabetes educators, mental health counsellors, lactation consultants and social workers
