Suggesting Updates to Database Listings

When viewing the full details of a community program, you will see the option to 'Suggest Update' just below the name of the organization and program. Clicking this option will allow a user to submit feedback to Community Connection's team of data curators. Users can also select 'Suggest Record' on top menu bar to suggest a new listing.

Description

Description (Service) Service that refers Ontarians who do not have a primary care provider (family physician or nurse practitioner) to one locally who is accepting new patients
  • priority is given to individuals who have greater health needs
  • waiting period depends on availability of family physician or nurse practitioner
  • Care Connectors are nurses who:
    • review health information provided upon registration with the program
    • follow-up with the individual with additional questions, if needed
    • work with local family health care providers to determine who may be accepting new patients in the community
    • once registered with the Health Care Connect Program an individual receives contact information for his/her assigned Care Connector
