When viewing the full details of a community program, you will see the option to 'Suggest Update' just below the name of the organization and program. Clicking this option will allow a user to submit feedback to Community Connection's team of data curators. Users can also select 'Suggest Record' on top menu bar to suggest a new listing.

Description

Description (Service) Assists older adults and seniors with mental health and addiction issues to live safely and maintain good health in an independent living environment to postpone or eliminate the need for institutional care. Accommodation for 54 residents in single and double rooms. 24 hour onsite services and supports respond to each resident's individual needs.

Support services include:
  • care coordination
  • intensive Psychogeriatric Case Management
  • personal care and assistance with the activities of daily living
  • full meal program provided
  • essential housekeeping, laundry, shopping and banking
  • medication support
  • escorts to appointments
  • 24-hour emergency response
  • crisis intervention and support
  • assistance in learning or relearning basic life skills
Social activation program offering stimulating activities and social opportunities such as group outings, in-house events, summer barbecues and holiday celebrations.

Vulnerable or at-risk men and women aged 55 and older living with mental health and addiction issues as well as multiple physical health concerns

Accepts referrals from a variety of sources including homes for special care, long-term care facilities, physicians, family members and HCC
